United eWay Online Campaign Form

This form is intended to be filled out by the United Way employee who is the primary contact for any eway campaign.  It should be filled out in consultation with the company representative responsible for the campaign.  Please submit the following form and all customized text and images to J.R. Logan (jrlogan@uwgnh.org) no later than 1 month prior to the kickoff of your campaign.

Best practice: have a conversation instead of sending via email (or ask a few questions via an email instead of a form)
Lessons: need multiple co. contacts to review (IT, HR, comm. person) -- mult. phases or pages for separate people
